However, it is likely that autoimmunity to cancer with central and peripheral nervous system involvement is more common and is not recognized because the symptoms are very mild or ascribed to other causes. Additionally, since these original incidence estimates were made, increased awareness, better definition of these conditions, and improvement in antibody detection techniques would probably make for a much higher frequency of paraneoplasia in cancer patients. The importance of recognizing a paraneoplastic disorder lies mostly in the early detection of an occult cancer but also in the specific treatment of the syndrome to reduce the associated symptoms and disability. The most effective treatment for paraneoplastic neurological disorders is treatment of the underlying cancer. The authors further describe the classical syndromes and onconeural antibodies reported to date. The typical initial symptom is dizziness, including vertigo; but unchecked, the condition will result in severe cerebellar dysfunction with dysarthria, double vision, and often profound truncal and appendicular ataxia. This also necessitates educating medical care providers about the importance of identifying and addressing symptoms early to ensure the best quality of life for the patient and family during the course of the disease.
